At South Wangaratta Medical Centre, we’re committed to helping you take control of your health with personalised, patient-centred care. A Chronic Condition Management Plan (CCMP) is designed to support anyone living with an ongoing health condition—this could include common chronic illnesses like diabetes, arthritis, or asthma, but also broader wellness goals such as weight management or improving physical function and wellbeing.
Your first CCMP appointment is a relaxed and informal chat with one of our experienced nurses. We’ll talk about your health goals for the year and work with you to create a tailored care plan. You’ll also see your GP briefly to review and finalise the plan.
To keep you supported and on track, we’ll schedule regular follow-up appointments every 3 months to check on your progress, offer encouragement, and make any necessary adjustments to your plan.
If needed, we can also provide referrals to allied health professionals—such as physiotherapists, dietitians, or exercise physiologists—to support your ongoing care.
The nurse and GP appointments are fully funded by Medicare, so there is no cost to you. (Please note: referred allied health services may have a gap payment depending on the provider.)
